To Schedule an Appointment:
Existing Patients: Please log into your patient portal with your User Name and Password. CLICK HERE to login in, or to re-set your password if you are not sure of your login details. If you are still unable to login, send an email to email@example.com to request a new portal invitation, and we will send you a new username and password as promptly as possible (this may take up to one business day). Once you have selected an appointment date and time, we will respond to either confirm your appointment, or to let you know if the appointment is already taken, within one business day.
New patients: Please CLICK HERE to to request an appointment. Someone from our office will contact you within one business day to let you know if the requested time is available, or to help you schedule an available appointment.
