Major Vision Plans Accepted: Vision Service Plan (VSP), EyeMed, Spectera, Superior Vision, SafeGuard, OptiCare, TML IEBP, United HealthCare Vision, Sun Life and Health, Guardian, Principal, Ameritas (LifeRe) and others. Unfortunately, we are not currently a participating provider with Avesis, Cole Managed Vision, Davis Vision or Eyetopia Vision plans.
Major Medical Plans Accepted: Medicare, BCBS, Cigna, HealthSmart, Boon-Chapman, Care Improvement Plus, Sterling Option I & II, TML IEBP, Principal Life, Ancillary Care Services (ACS) clients and many others. We accept all plans supplemental to Medicare. We are currently out-of-network providers for United HealthCare, UMR, Aetna and Humana major medical plans. Unfortunately, we are currently not a participating provider for Medicaid or CHIPS.
